Duplication Indemnity Agreement
This agreement is required on all orders to indicate that we are not reproducing copyrighted material. You may make one copy of any copyrighted material for archival purposes. This form also gives us your company information for billing and shipping purposes.
Credit Application
Our standard credit terms are C.O. D. If your company would like to be considered for Net 30, please completely fill out the credit application and fax to 303-449-7553.
